Safety Issue because sharing is caring
This weekend I was swapping my winter tires on one of our cars and while torquing a wheel nut I ended up breaking my nose and being concussed I had it set to 104/lbs/ft and it was rated much higher and had both hands on it and was crouching next to car on the third wheel. Next thing I know, I did a faceplant, nose first into the rear passenger window (Passat wagon). Luckily, a friend of mine was there to help me. "Man I thought you were dead" "there was blood everywhere" as I was regaining concsiousness The tool simply failed

Wow! That's quite the punch in the face! That sounds worse than the time i was smartly supervising a buddy of mine while he was installing my brake spring with a vice grip and smashed me in the face with it. I hope you recover soon! Did you really break you nose and get a concussion?
Lessons learned: No more drum brakes and vice grips in the same sitting and wheel nuts only need to be torqued at 70-80 ft lbs.

Sorry, I misread the original post.
I have witnessed others popping themselves I the face when pulling torque back into themselves.
I guess in you're case our was a matter of lost balance due to the surprise of the sudden lack of resistance.
I HATE when that happens.

sounds like your lugnuts were put on with an impact wrench. I never push that hard on a wrench for this exact reason. I go and get a piece of pipe and let it do the work.

FWIW, I'm on serious concussion number five (though that was probably twenty years ago). I'm sure there's been one hell of a cumulative effect, but since I can't tell (b/c I've been too concussed) I don't worry about it.
If you were out cold, make sure to take it easy for a bit and let that brain of yours heal.
